This form authorises the BLA to complete a background check for individuals relating to a registration.

You will need to provide 100 points of identification and certified copies of the relevant documents with this form. The form outlines who can certify the documents as true copies.

Permission Application Form

You must complete this form if you are disqualified and need to apply for permission.
You cannot apply for permission online. You need to print the form and complete it by hand and post it to the BLA with the application fee.
