Company Document Checklist
Each director must submit the following documents.
Identification: Certified copies of 100 points of identification.
Criminal history: A completed Criminal History and Other Records Check Consent form.
The company application allows one director to apply for an individual licence at the same time as the company licence. If the company does not have a licensed director then at least one director must apply for an individual licence. Other directors wishing to be licensed must complete a separate individual application.
