Use this form to apply for registration as a Credit Provider, if you are a Company.

  • Complete the application form online.

What if I don't have a scanner or credit card?

  • Download the form to your computer. The following options are available from within the form:
    • If you want to type the required details into the form, select Save Form, to save the form to your computer.
    • If you want to complete the form by hand, select Print Blank Form to print out a blank form in full.
  • Complete this form.
  • If you completed the form online, select Print Form to print the completed form.
  • Include hard copies of the required documents.
  • Pay the application fee by cheque, money order or credit card (Mastercard, Visa and Amex).
  • Post the application or lodge in person to BLA.

If I do have a scanner, what do I need to scan and attach?
The form lists exactly what you need to attach to support your application.  As restrictions apply on the size of attachments, ensure that you scan documents at a low resolution and in grey scale to keep the attachment within allowable guidelines. If you are attaching large documents, it is recommended that you complete the application in hard copy and lodge by post, rather than online.
